Our team will work closely with families in getting the care needed for their child. This can include medical care, mental health treatment, and other social services. Home Health Care Managers can help families understand their child’s health and provide additional information to manage conditions and connect them to doctors, peer advocates, treatment centers, housing related supports, school services, community programs, and resources.
