ADAPT COMMUNITY HABILITATION PROGRAM
The Community Habilitation Program at ADAPT Community Network is a person-driven, outcome-based service tailored to the participant’s needs, wants, and preferences through the implementation of valued outcomes and goals.
HOW IT WORKS
We work with program participants, their families, or a chosen network of support by:
- Providing one-on-one assistance to each participant, whether they live in a family setting, Certified Residence, or independently
- Creating and monitoring comprehensive habilitative plans
- Empowering each individual to obtain their goals, hobbies, and pursue their interests
- Working with individuals, including children, adults, and aging populations, to increase their independence
- Supporting individuals to be active participants in their communities

- Adaptive skill development which includes problem solving, organization, maintaining a household, and more
- Support for independence in living and traveling
- Development of social and informed choice skills
- Guidance in accessing one’s community
- Training in self-advocacy and communication skills
- Supporting educational goals, homework , money management, budgeting, and technology
How to Access Community Habilitation?
To apply for Community Habilitation services with ADAPT you must be determined eligible through the Office for People with Developmental Disabilities (OPWDD). You or your loved one must have a Care Coordinator and be enrolled in Home and Community-Based Services (HCBS) Waiver Services.
