Doorways to Independence

Home Modification Services

The Doorways to Independence program assists persons with physical disabilities, the elderly and medically frail with their home modification and adaptive equipment needs. Funding is available to cover the cost of wheelchair ramps, stairlifts, accessible bathrooms, light weight strollers, adaptations for those who are deaf or have limited hearing, etc. Full project coordination is offered for eligible applicants, including home assessments, scope of work development, obtaining vendor/contractor estimates and project management. Eligibility

Access to Home & Access to Home Medicaid Grant
Service available in all five boroughs
Applicant could be living independently or with family
Home Owners & Renters may qualify for assistance
Must have a permanent physical disability or have substantial difficulty with ADL’s due to aging
Total household income may not exceed 80% of area median income
Allocation of funds may vary, however the project cap is 25k
Family Supports and Services Grant OPWDD
Service available in all five boroughs
Applicant must be residing with family
Home Owners & Renters may qualify for assistance
Must be diagnosed with a developmental disability and be approved for OPWDD services
There is no income requirement to qualify for this assistance
Allocation of funds may vary, however the project cap is 6k
