SUMMARY

Under general supervision, is responsible for providing evaluation, diagnosis and therapeutic intervention in accordance with ADAPT Community Network policies, the regulations and requirements of applicable regulatory agencies and the standards of the profession. Monitors treatment plan and progress, provides training to residential staff, and conveys techniques to others.


ESSENTIAL DUTIES AND RESPONSIBILITIES

The duties and responsibilities of the Registered Nurse - Residential will include but are not limited to the following:

  • Serves as a member of interdisciplinary team and is responsible for coordinating and following through on provisions of health care services for assigned residences.
  • Conducts training for staff in medical administration, personal hygiene, basic first aid, dental hygiene, infection control, adaptive equipment, vital signs, signs and symptoms of illness and other in-services as may be necessary to meet needs of individuals in each residence.
  • Handles control of communicable diseases and infections through identification and assessment, reporting to consulting physician and implementation of appropriate protective and preventive measures and staff training regarding infection control.
  • Develops, supervises implementation of, reviews and revises each resident’s written health care plan that is developed and implemented according to instructions of a physician and assessment. This plan should be reviewed and revised as needed.
  • Integrates therapeutic intervention techniques into programs or home of person served through consultation with parent/guardian and staff at team meetings, case conferences or through other informal contacts.
  • Develops a written plan for each person we support to provide nursing services as a part of total habilitation programs.
  • Participates in interdisciplinary team meetings and annual reviews as required by each division.
  • Monitors required staff health records and provides tests in accordance with regulatory agency requirements.
  • Provides immediate first aid with referral to outside emergency services or private physician.
  • Maintains a tickler for all appointments. Schedules appointments and follows up on physicians’ recommendations. Makes recommendations as changes occur.
  • Keeps up to date and accurate records of examinations, immunizations, illnesses, accidents and other pertinent data via progress notes.
  • Administers medications when appropriate and supervises AMAP certified staff in medication administration. Supervises all medication prescribed for individuals in accordance with agency policies.
  • Provides supervision and training for Licensed Practical Nurses and/or less experienced Registered Nurses during their tenure at agency.
  • Monitors progress for self-administration of medication.
  • Obtains consents for surgery of residents.
  • Reviews all diagnostic work and consults.

QUALIFICATIONS

To perform this job successfully, an incumbent must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.   • License and current registration as a Registered Nurse in the State of New York.
  • CPR Certification.
  • Ability to read, write, speak and understand English.
  • Good oral and written communication skills.
  • Good organizational skills.
  • Interpersonal skills necessary to interact effectively with coworkers, employees, individuals, families and outside contacts.
  • Must be able to maintain the strictest confidentiality.
  • Knowledge of equipment typically used by persons receiving services, including wheelchairs and adaptive devices.

EDUCATION and/or EXPERIENCE

  • Graduate of an accredited program of professional nursing.
  • At least one year of experience as a Registered Nurse in the delivery of community-based health services to individuals with developmental disabilities.

The duties and responsibilities of the Licensed Practical Nurse - Residential will include but are not limited to the following:

  • Serves as a member of interdisciplinary team and is responsible for assisting Registered Nurse in coordinating following through on provisions of health care services for assigned residences.
  • Keeps progress notes under supervision of registered nurse for each client and summarizes for quarterly, semi-annual and annual reviews.
  • Provides health care to individuals receiving services as instructed by registered nurse. Provides direct hygiene and health instruction to individuals receiving services and families as required.
  • Handles control of communicable diseases and infections through identification and assessment, reporting any finding to registered nurse and consulting physician, implementing appropriate protective and preventive measures and monitoring infection control practices.
  • Provides first aid to persons in need of immediate care, with referral to registered nurse or outside emergency services as necessary.
  • Assists physicians in medical examinations as needed.
  • Maintains a tickler for all appointments. Schedules appointments and follows up on physicians’ recommendations.
  • Keeps up to date and accurate records of examinations, immunizations, illnesses, accidents and other pertinent data via progress notes.
  • Administers medications when appropriate. Supervises storage of medications and assists in oversight and ordering of medication.
  • Assists registered nurse in maintaining AMAP re-certification files.
