What is Health Home Care management
A Health Home is not a place, it is a person who can help you find services that are right for you and your family. Health Home Care Managers work together as a team with children/youth and their families and service providers to make sure they receive the care and services they need to stay healthy. All Children eligible for HCBS can get care management services through the Health Home care management program.
*Enrollment in HCBS requires participation in the Children’s 1915c waiver.
A care manager will support you and work closely with you to ensure that you get the care you need and want for your child, and that all care is well coordinated. This can include medical care, mental health treatment, and other social services. Care managers can help you understand your health and provide additional information to manage conditions and connect you to doctors peer advocates, treatment, housing related supports, resources to access food, transpiration to appointments, school related services and other community programs.
