ADAPT Community Network Receives Grant to Support Home Safety for Older Adults

ADAPT Community Network is proud to announce a grant received by the U.S Department of Housing and Urban Development (HUD) that will help fund our Older Adults Home Modification Program (OAHMP).  With this grant, eligible applicants may qualify for up to $15,000 per household for home modifications and improvements.

What is the Older Adults Home Modification Program?

The Older Adults Home Modification Program supports older adults in aging safely at home by providing small-scale, non-structural home modifications that reduce fall and injury risks. As adults age, it’s important that they do so successfully. ADAPT’s goal is to maintain their independence while remaining safe and comfortable in the comfort of their homes.

Who is eligible for the program?

To be eligible for the program, you must:

  • Be an adult who is 62 years or older
  • Reside in one of the five boroughs of New York City
  • Have a household income at or below 80% of the Area Median Income (AMI)
  • Be a homeowner or tenant with landlord approval

How can I apply?

The OAHMP is currently accepting applications. To get started, you must apply to the program through ADAPT’s application form.

If you’re deemed eligible, an occupational therapist may assess your household and the improvements needed to support safer daily living. They provide tailored recommendations, along with guidance and training to help you use any modifications effectively. Their work is centered on the applicant, considering each person’s needs, abilities, and preferences when suggesting change.  

Once the project is approved, ADAPT will oversee the contract and cover the costs. The Occupational Therapist will also conduct follow‑up visits to ensure the modifications are working as intended and often partner with other professionals to make sure everything is implemented properly. 

Throughout the entire process, ADAPT guides you step by step through the possible modifications, so applicants understand every stage of the process.

What is covered under this grant?

The Older Adults Home Modification Program covers a wide range of home modifications. Some examples of OAHMP maintenance repair activities include:

  • Ramp Installations
  • Installation of grab bars or handrails
  • Repair or replacement of lighting fixtures for better visibility
  • Repair of minor plumbing or electrical issues that affect safety
  • Installation of smoke or carbon monoxide detectors
  • Door lever handle replacements

The OAHMP can aid in activities of daily living as well as home modifications. Aids for daily living activities include everything from a shower chair, all the way up to a stairlift. Multiple modifications can be done within one project depending on how much the total cost is.

There is currently no application deadline; however, funding is limited and will be distributed on a first-come, first-served basis. Those interested are encouraged to apply as soon as possible.
