- Remote health assessment and coordination services provided by licensed clinicians trained in I/DD care.
- Technology-based systems that allow individuals to receive monitoring or assistance from trained staff when appropriate.
Eligibility
- Must be OPWDD-eligible and HCBS Waiver-enrolled
- Must live in a non-certified residence within the five boroughs of NYC
