As our community continues to be affected by the COVID-19 outbreak, many of our programs have been affected. We will be keeping this page up-to-date with how our programs are currently operating, and with resources for those affected by the changes. We hope to resume normal operations as soon as is safe.
Family Support ADAPT’s Family Support Department is striving to maintain a high level of continuity and support during these uncertain times. We continue to provide resources and referrals through our Project Connect information line (877-827-2666). Please clearly leave your name and contact information, or email us at ProjectConnect@adaptcommunitynetwork.org. A Project Connect Coordinator will get back to you with the support information requested.
We recognize this is an unprecedented time for everyone, and understand how this changing situation impacts the people we support and their families. In response to the current situation, we have implemented new ways to provide support services, which include Telehealth technologies. These precautionary measures have been taken to protect the people we support as well as staff. We are happy to announce we are taking applications for all of our supports and services including home modification, family reimbursement, housing advocacy, Community Habilitation, Independent Living, and Rent Subsidy Service(ISS). The processing time for applications may be extended, but we will update you regularly on the status. Our senior leadership team is continuing to monitor the rapidly evolving situation and quickly respond as conditions change. We are committed to providing the supports and services you depend on.
Healthcare Article 28 Clinic services are currently being provided through Telehealth and telephonic means. Our Primary Care providers have access to our Electronic Medical records, so if you have a medical need during this crisis, they can review your record (including your health history and medications) to best advise and treat you. If you have a medical need, question, or need a refill for your medications, please call your clinic location and follow the telephone prompts. We will do best to call you back within the hour. Our after hour service remains in operation after 5 pm.
Article 16 Clinic clinics are currently providing Mental Health Counseling by Telehealth or Telephonic means. For additional information on how to receive Mental Health Counseling or about any other A16 services, please contact the Treatment Coordinator in your borough.

Residences In our residential locations, we are continuing to follow best practices for infection control measures, routinely cleaning and taking steps necessary to minimize the spread of infection. We are ensuring all our staff have the appropriate Personal Protective Equipment (PPE), so they remain safe and limit exposure. Our dedicated staff is continuing to support people both in ADAPT’s residential locations as well as those living with family or on their own. Please don’t hesitate to reach out to your director or our Project Connect information line (877-827-2666) or email us at ProjectConnect@adaptcommunitynetwork.org so we can best support your needs. We are continuing to follow the guidance and direction of the Center for Disease Control, the NYS Department of Health, the NYC Department of Health and Mental Hygiene, as well as the Office of Persons with Developmental Disabilities. Thank you for your continued cooperation while we are required to restrict the visitation of all visitors and non-essential health care personnel during this time. Your Program Director will continue to work with you on ways you can stay in touch with your loved one via video conferencing and phone calls. We will continue to evaluate the situation and will remain in contact as the situation evolves.
